All t-shirts are adult sizes. Each rostered player will receive a free t-shirt. The league will have a limit of 16 shirts per team. Coaches may receive a free shirt if the total is not over 16. Your roster must be returned with this order form.

This form and your league fee MUST be returned To Ray Knight by October 17th, 2004 for you to receive T-Shirts for your Team.
